By purchasing a consultation you are guarnateed I will  e-mail you within 1 business days.  Attached to the e-mail will be questionnaire forms for you to complete and return.
Within 2 business days of receipt of your completed forms I will contact you to set up a 30-45 minute phone appointment, to discuss your concerns, my evaluation and recommendations.  I will schedule you for a follow up appointment in 2 weeks, which is included in this initial $35 fee.
